While charges are payable at treatment time, we accept cash, credit cards, CareCredit, and personal checks for your convenience. Please get in touch with our office manager before receiving services if special arrangements are needed.

  • 1 set of Check-up X-rays (4 Bitexing X-rays & 2 Periapical X-rays)
  • Routine Cleanings: 2 per year (Excluding scaling and root planning and gingivitis scaling)
  • 2 Fluoride Treatments
  • All at no additional Cost

Perio Plan

$ 549 Per Person Yearly

As a precaution, we strongly suggest contacting our near Jacksonville dental office to confirm your benefits if we do not participate in a particular plan.

The bottom line is that our team will submit all insurance claims to help you receive full insurance benefits. However, patients are personally responsible for their accounts, and we encourage you to reach out to our office if your policy has yet to be paid within 30 calendar days.
